Aging & Disability
Resource Connection
The Aging and Disability Resource Connection (ADRC) initiative is a statewide effort designed to reduce barriers for older adults and people with disabilities of all ages. Historically, these groups have had to navigate separate and sometimes disconnected service systems, even though their support needs often overlap throughout the lifespan. ASL Interpreting
ACC maintains an American Sign Language (ASL) Interpreter Registry to ensure communication access for meetings, medical appointments, and other essential interactions. Consumers, businesses, medical providers, community organizations, and others can request an ASL interpreter to ensure individuals who are Deaf or hard of hearing have full access to communication.

Assistive Technology
ACC helps identify and obtain devices that support greater independence. Our Assistive Technology (AT) Coordinator can assess your needs, locate equipment for trial, and guide you through the funding process to secure the items you need. Examples of assistive technology include wheelchairs, walkers, canes, speech devices, laptops, specialized software, and more.
Benefits Counseling
ACC provides support with understanding, applying for, and appealing public benefits. Our Benefits Specialists can guide you through programs such as unemployment, SSDI/SSI, Medi-Cal, Medicare, and others. We know the benefits process can be complex and frustrating, and we’re here to help you navigate it with confidence.
Blind & Low Vision Services
ACC’s Blind & Low Vision program provides services for adults age 18 and older who are experiencing vision loss or blindness. Whether you’re noticing changes in your vision, need simple adaptations like tactile markers on appliances, or require more in-depth support, we are here to help. Our programs can assist with adaptive devices, orientation and mobility training, independent living skills, and a wide range of tools and strategies to support your independence.
CalAIM
ACC provides CalAIM Community Support Services for eligible Medi-Cal members. These services include housing support, transition assistance, and home modification resources. Specific CalAIM offerings include Housing Transitions and Navigation Services (HTNS), Housing Deposits (HD), Housing Tenancy and Sustaining Services (HTSS), and Nursing Facility Transition.
Community Integration
ACC’s Community Integration Specialists support safe and successful transitions from institutional settings back into the community. Whether you are leaving a skilled nursing facility, hospital, board and care, or another institutional environment, our team can help you navigate the often overwhelming return home. Services may include assistance with locating housing, securing home modifications, applying for public benefits, and coordinating other supports needed for community-based living.
Emergency & Disaster
Preparedness Services
ACC’s Emergency and Disaster Preparedness Program is designed to provide peace of mind during emergencies and natural disasters. Our Emergency Preparedness Services Managers will help you to develop a personalized plan, this may include creating an evacuation strategy, enrolling in the Medical Baseline Program, or obtaining/creating an emergency kit. Independent Living Skills
(ILS) Training
Independent Living Skills are the everyday skills that help individuals increase or maintain independence. ACC provides training in areas such as applying for an ADA transit ID card, enrolling in school, managing a bank account, and other essential tasks that support self-sufficiency.
Information & Referral
Information and Referral (I&R) is one of ACC’s core services. When you contact ACC, you will be connected with one of our bilingual Community Information Coordinators, each located in one of the three counties we serve. They will conduct a conversational needs assessment and connect you with the appropriate staff member for the services you’re seeking. If you request support that ACC does not provide, we will make a warm hand-off to the organization best suited to help.
Peer Support
ACC offers services and support grounded in lived experience. As individuals with disabilities ourselves, we understand the challenges and perspectives of the people we serve. At least 51% of our staff and board members are individuals with disabilities, and currently, 80% of our team reflects this commitment. This representation ensures our services remain authentic, informed, and responsive to the needs of our community.
Personal Assistance
Get connected to the services you need to live more independently—such as help with cleaning, cooking, bathing, and other daily tasks. ACC’s Housing & Support Specialists can assist you with applying for personal assistance programs, including In-Home Supportive Services (IHSS), ACC’s Personal Assistant Registry, or other available options to ensure you receive the support that best fits your needs.
Systems Change Advancement
ACC is dedicated to advancing disability rights and improving community systems through both individual and collective advocacy. We provide self-advocacy training to help individuals understand their rights, build confidence, and navigate complex service systems. In addition, ACC facilitates a consumer advocacy group, creating a space for individuals with disabilities to raise concerns, identify community barriers, and work together toward solutions.
Our staff also actively engages in state and local systems change efforts, collaborating with agencies, coalitions, and policymakers to promote accessibility, equity, and inclusion across our region. While ACC supports advocacy at many levels and empowers consumers to be strong voices in their communities, we do not provide legal services or legal representation.
